Pneumoperitoneum is the accumulation of free air or gas in the abdominal cavity, more specifically in the peritoneal cavity. This can happen intentionally, during surgery for example, but also due to perforation of the digestive tract due to disease or trauma… or due to sexual activity.

An American team (Stanford University) reports the classic case of a young woman who came to the emergency room with recurrent chest pain. The X-ray showed free air under the diaphragm. After the doctors had done a thorough examination and thoroughly discussed the patient's sexual history, vaginal insufflation during sexual activity was found to be the cause of non-surgical pneumoperitoneum.

How does air get from the vagina into the abdomen?

Few women (and men) are aware that there is an open passage between the vagina and the abdomen. The air inhaled through the vagina travels to the uterus and then to the fallopian tubes, which end in two openings. The high pressure during insufflation can dilate the genitals and push a large amount of air into the abdominal cavity. It can take several days for the gas to be absorbed. Women often do not associate the occurrence of pain with the sexual act, but that is still the cause.

When should you worry?

If you experience abdominal pain after such sexual activity, discuss it openly with your doctor. Postcoital pneumoperitoneum should not be viewed as a bizarre “sexual accident,” but as a serious condition. To avoid unnecessary medical tests, specialists advise doctors to always consider vaginal air insufflation if a patient presents with atypical pneumoperitoneum that is not associated with a disease or surgical procedure.

The consequences of blowing into the vagina can be very serious for pregnant women because of the risk of embolism. As for “oral-genital” stimulation without insufflation, you should remember that there is a risk of transmitting a sexually transmitted disease (STD).
