Diablo IV Lead Class Designer Adam Jackson took to Twitter to explain in more detail how and where to obtain the rarest unique items in the game.
In total there are currently six, including:
- Doombringer (sword)
- The Grandfather (two-handed sword)
- Ring of Starless Skies (ring)
- Andariel’s Visage (helm)
- Harlequin Crest (helm)
- Melted Heart of Selic (Amulet)
According to Jackson, they drop from enemies whose level is 85 or higher. There isn’t a particular location to find each of them, with all of them being found where normal unique items drop.
Jackson said that you can deduce that you’ve found one of the rarest items just by looking at its power, which needs to be 820. But he made it very clear that the chance of finding them is exceptionally low, so good luck!

1. They can drop from level 85+ enemies
2. You get them anywhere you can get a regular Unique, and they always drop at 820 ipower
